Cumberland Surgical Arts and Associates, PLLC is open Monday through Friday from 8:00 AM to 4:30 PM.

Evening hours and weekend hours are available for emergencies for current patients that have recently undergone surgery. We will schedule your appointment as promptly as possible.

Your Information

To schedule an appointment, we require the following information

• Reason for the appointment
• Name of referring provider
• Insurance plan name, ID number, and group number
• Insurance Subscriber’s name and date of birth
• Patient’s social security number
• If a patient is under 21 years of age, we will need patient’s and guarantor’s social security numbers.
• Patient’s contact information, including phone number and email address for appointment confirmation

Referrals

Our doctors request a current referral from your physician or dentist that is no older than six months. The referring office can fax your referral to our office in advance. Our fax number is 931-552-3243. The referral is necessary for your consultation appointment.

Appointments and Surgery

Each patient must have a consultation appointment prior to scheduling surgery. We cannot hold surgery appointments. All surgeries are scheduled after the doctor has created a surgical plan for you.

We try our best to stay on schedule to minimize your waiting. Due to the fact that Cumberland Surgical Arts and Associates, PLLC provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
